We are no longer taking applications for the Academic Year 2025-2026

Please fill out the application as soon as possible as seating will be limited.  Please provide accurate information. 

This application is our only means of identifying you as interested in the Life Science Academy and we can use your contact information to help you through the other steps of our application process as necessary. Students are encouraged to complete the testing/application process by the end of April.  Due to high demand, applicants are encouraged to apply as early as possible as enrollment is limited.  The earlier you complete the application process, the smoother your high school scheduling will be. When you schedule for high school, please indicate that you want to include Community Campus (Life Science Academy) among your courses. The finalized enrollment goal is the end of April; however, the enrollment opportunity will be open only as long as there are spots. You should receive confirmation once you submit the application below and we will get in contact with you as soon as we receive your test scores. 
An application requires 3 things in order to be considered for admission.  Students must (1) complete this application form, (2) submit an ACT score to Ms. Jerome, and (3) provide a core teacher name for a recommendation.  Students will not be considered until these 3 things are complete.

    The information below will allow us to contact your core teacher to write a short recommendation letter on your behalf.  Please contact the teacher in advance and make sure they are willing to write a recommendation letter for you.  Please tell the teacher we will contact the teacher.

    SB150 Waiver - Parent or Guardian Must Complete this Section

    Your student’s enrollment includes Advanced Placement (AP), Dual Credit, and/or Life Science Academy classes.  These classes provide opportunities for students to participate in college level courses. 
     
    In compliance with KRS 158.1415 and KRS 160.345, all schools are required by law to notify parents and receive parental permission when the topic of human sexuality is addressed within the course content.
     
    Many of our AP, Dual Credit and Life Science Academy classes potentially cover topics typically not addressed in traditional high school courses related to human sexuality. This letter serves as written notification to you as guardian that due to enrollment in AP, Dual Credit, and/or Life Science Academy classes your child may be enrolled in a course that covers human sexuality. For example, AP Biology and health science related courses cover reproduction. 
     
    Per DCPS board policy, you must provide written consent for your student to participate in such courses.  If we do not receive your permission, your student’s enrollment will need to be adjusted.
     
    As guardian, you may also opt to further review the content of the courses your student is enrolled in.  To do so, email Dr. Chad Wilkerson ([email protected]) with a request for further view of the course. 
     
    I acknowledge that my student is enrolled in AP, Dual Credit, and/or Life Science Academy courses that potentially cover topics related to human sexuality.  By typing your name below,  you give consent for your student to participate in such course(s).
    To apply to the Life Science Academy, we only need students to submit this application one time.  There is no situation where a student should submit this application multiple times.  If a student would like to update any of the information on their application at a later time, please send an email to Dr. Wilkerson ([email protected]).
