Recommendation Letter Request Form

In order for me to write the best letter possible, I will need you to complete this form.  I need at least 2 weeks notice to write an effective letter.  


I need all of the following pieces of information so I can write a strong letter for you.
  1. When is the submission deadline for my recommendation letter.
  2. What is your current GPA
  3. List some of your more important classes (with grade in each class).  This would be classes that support what you are applying for.  For example, if you are applying for a science or healthcare program or science scholarship please list relevant science classes. 
  4. List your extracurricular activities (sports, clubs, etc)
  5. Most importantly, you need to list all of the volunteer work you have done (including benefit and toy drives through LSA) and provide start and stop dates or how long you have done these.
  6. State why you think you would be a good candidate for this scholarship or program. 
  7. Provide a brief summary of what this program or scholarship is about.  What are they looking for?
  8. How do you describe your ability to work in group settings?  Do you like working with others?
  9. What is your current career goal?
