LSA Application
We are no longer accepting applications for the 2021-2022 academic year.  Thank you for your interest in the Life Science Academy.

Please fill out the application as soon as possible as seating will be limited.  Please provide accurate information. 

This application is our only means of identifying you as interested in the Life Science Academy and we can use your contact information to help you through the other steps of our application process as necessary. Students are encouraged to complete the testing/application process by the end of April.  Due to high demand, applicants are encouraged to apply as early as possible as enrollment is limited.  The earlier you complete the application process, the smoother your high school scheduling will be. When you schedule for high school, please indicate that you want to include Community Campus (Life Science Academy) among your courses. The finalized enrollment goal is the end of April; however, the enrollment opportunity will be open only as long as there are spots. You should receive confirmation once you submit the application below and we will get in contact with you as soon as we receive your test scores.

    We will make every effort to place you in the section of your choosing but cannot make any guarantees- first come, first served and seating is limited. Which section would you prefer?

    At this point the afternoon section has a couple of spots open.  If you need the morning section you should select the wait list option.


    Application Tier

    Use the dropdown box below to select the situation that best applies to you.

    Tier 1: Composite score ACT 18 or above & 3.5 GPA (Automatic admission) **NO essay or teacher recommendation needed
    Tier 2: GPA 3.5 and above, essay requirement and core teacher recommendation.
    Tier 3: GPA 3.0 and above, essay requirement, core teacher recommendation and Google Meet interview.

    For Tier 2 and Tier 3 Applicants Only:
    Core Teacher Contact Info (Math, Science, English/Reading Teachers only)

    The information below will allow us to contact a core teacher to write a short recommendation letter on your behalf.  Please contact the teacher in advance and make sure they are willing to write a recommendation letter for you.  Please tell the teacher we will contact the teacher.

    Testing Information

    If you plan to take or retake the ACT in the Future.

    Please click below to confirm the following: I understand that if I take the ACT, my ACT scores MUST be sent to Ms. Amanda Jerome and that this WILL NOT happen automatically. Once I receive my scores I will scan the official document and email it to Amanda Jerome - College/Career Readiness Coordinator [email protected]. I will call Ms. Jerome with ANY questions regarding my test scores @ (270) 852-7000.
